Job Description
The LPN at DAVID HOME HEALTH NURSING SERVICES LLC plays a critical role in facilitating dynamic and supportive nursing care tailored to individual patient needs. As a pivotal member of our healthcare team, you will work closely under the supervision of registered nurses and other healthcare professionals. This position demands a professional who is adept at balancing multiple patient needs and committed to delivering the highest quality of care in every interaction.
Duties and Responsibilities
- Assess and record patients’ vital signs and medical information, updating healthcare team leaders on patient condition and progress.
- Administer prescribed medication to patients as directed by the attending healthcare provider and in accordance with all regulations.
- Monitor patients' health status and notify appropriate medical personnel of any significant changes.
- Assist in the implementation of individualized care plans tailored to each patient’s specific needs.
- Provide basic patient care and treatments, such as dressing wounds, treating bedsores, giving enemas, douches, alcohol rubs, massages, and maintaining personal hygiene.
- Operate and monitor medical equipment and assist with the use of devices like wheelchairs or respirators.
- Collect samples for testing and perform routine laboratory tests on premises.
- Educate patients and their families on health care, including providing information on disease prevention, health maintenance, and management of chronic conditions.
- Prepare and maintain accurate patient records, charts, and documents to support the healthcare team’s efforts to ensure excellent patient care.
- Support a compassionate environment by providing emotional, psychological, and spiritual support to patients and their friends and families.
- Collaborate effectively with other healthcare professionals to ensure the comprehensive delivery of patient care services.
- Maintain a safe and clean working environment by complying with procedures, rules, and regulations, and calling for assistance from other healthcare support if necessary.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of employment.
- Minimum of 1 year experience in a clinical setting, preferably in home health care or a similar environment.
- Proficient in both oral and written communication skills.
- Ability to deliver effective patient care through adherence to nursing care standards and protocols.
- Demonstrated ability to record and report symptoms and changes in patients' conditions.
- Strong interpersonal skills with a proven ability to work collaboratively in a multidisciplinary team environment.
- Basic understanding of patient care and medical terminology related to home health care.
- Ability to operate or quickly learn to operate a wide range of medical equipment.
- Highly organized, with the ability to multitask and prioritize work under pressure.
- Physical endurance is necessary to perform demanding tasks daily.
- Empathy and a person-oriented approach to health care provision.
- Current CPR certification is preferred.
