The Hospice Registered Nurse (RN) provides compassionate, patient-centered care for individuals with terminal illnesses and their families. As a key member of the hospice team, you will deliver quality end-of-life care in a supportive and empathetic manner, focusing on comfort, symptom management, and emotional support. You will work collaboratively with physicians, social workers, chaplains, and other healthcare professionals to ensure a dignified and peaceful transition for patients.
Schedule: Full-Time, Weekdays, 8am - 5pm, On-Call

Primary Responsibilities and Duties:
- Conduct thorough assessments of patients; physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the patient, family, and other hospice staff.
- Administer medications, manage pain and symptom control, and perform medical procedures as needed.
- Monitor and document patient progress, changes in condition, and response to treatments.
- Educate patients and their families on disease progression, care options, and symptom management.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.
- Offer emotional and psychological support to patients and families, addressing their concerns and facilitating discussions about end-of life care.
- Coordinate with other members of the hospice team to ensure comprehensive and continuous care.
- Advocate for patients’ preferences and needs, respecting their values and wishes.
- Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
- Participate in on-call rotation as required to provide care and support after hours.
- Perform all other duties assigned.
