Sage Group HealthCare Staffing is seeking a Hospice RN for travel/contract positions in Columbus, OH. We are accepting both local and traveler candidates.

Position Details:

Position Name: Hospice Registered Nurse 

Location: Columbus, Ohio 

Specialty: Hospice 

Start Date: ASAP

Employment Type: Full-time, Travel, Contract, Temporary

Contract Length: 13 weeks contract

Pay Rate: Range $2080 to $2120 Weekly Gross pay based on a 40 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Unit: Home setting

Shift available: Days

Shift timings:  10-hour Days, 8.00 am to 8.00 pm

Schedule: 40 hours/week guaranteed. 4 X 10-hr shifts, Fri/Sat/Sun/Mon


Position Requirements:

  • Registered Nurse: 2 years - Required
  • Recent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years – Required
  • Hands on Home Care Home Base (HCHB) charting experience – Required  
  • Must have valid Driver’s License and Personal Auto Insurance

Education, License and Certification Requirements:

  • ASN or an equivalent – Required
  • BSN or an equivalent – Highly preferred
  • RN License for State of Ohio or Compact - Required
  • BLS Certification from AHA or American Red Cross - Required

Job Description:

  • Provide and coordinate care for assigned Hospice patients 
  • Complete admissions, visits and manage assigned case load 
  • Develop and implement nursing care plans for patients as assigned.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting
  • Territories: 60 miles from Dublin office
